Japan has a medical insurance system that all citizens can use. Therapies such as occupational therapy and physical therapy can also be performed through insurance treatment. In addition, there is a long-term care insurance system, and the elderly can use it.

Japan’s Long-Term Care Insurance System (Kaigo Hoken Seido)

Japan’s Long-Term Care Insurance System is a public insurance program established in 2000 to collectively support the long-term care needs of the elderly across society.

There are two types of insured persons under the Long-Term Care Insurance System, categorized by age:

  • Category 1 Insured Persons: Individuals aged 65 and over.
    • They can use long-term care services when they receive certification of requiring long-term care or support, regardless of the cause.
  • Category 2 Insured Persons: Individuals aged 40 to 64 who are enrolled in a medical insurance plan.
    • They can use long-term care services when they receive certification of requiring long-term care or support due to specific age-related diseases (tokutei shippei).

Steps to Access Long-Term Care Services

  1. Application: To use long-term care services, you must first apply at your local municipal office.
  2. Certification of Need for Care: After applying, your physical and mental condition will be assessed, and you will be certified for a level of care need (Support Level 1 or 2, Care Level 1 to 5).
  3. Care Plan Creation: Based on your certified care level and considering your preferences and your family’s, a care manager (care manager or kaigo shien senmonin) will create a care plan (long-term care service plan).
  4. Service Utilization: Based on the care plan, various long-term care services can be utilized, such as home-visit care, day services, short-term stays, and institutional care.

Japanese OT

In Japan, there are both private and public insurance systems. The public health insurance generally covers almost all medical treatments. This comprehensive coverage means that Japanese occupational therapists can provide all necessary therapies to their clients.

 日本においても、プライベート保険と公的保険があり、公的保険で医療はほぼ全てカバーすることができる。そのため、日本の作業療法士はクライアントに必要なセラピーは全て行うことができる。
